Creating a Mid-Semester Feedback Survey

Mid-Semester feedback enables faculty to fine-tune a course that is already in progress. Gathering this type of feedback sends a message to students that you care about their success in the course. 

To get honest feedback from students, make the survey anonymous.  

To assist you, the Center for Teaching & Learning has created TWO global question banks that consists of mid-semester related questions - one for face-to-face courses and one for online/blended courses. You may use all, some, or none of the questions in these question banks.


Step 1: Create a New Quiz

Select Quizzes [1] in your left course navigation, then the "+ Quiz" button [2]Note: If you are prompted to select 'Classic Quizzes' or 'New Quizzes,' select 'Classic Quizzes.'

Step 2: Configure Your Quiz as a Survey

The Details tab will open, give your quiz/survey a title [1], add directions [2], select ungraded or graded survey from the drop-down menu [3]—in this case ungraded survey—and whether or not to keep submissions anonymous [4].

Assign To - Multiple Sections

If you have merged multiple sections and would like to give them different due dates, click "X" to remove 'Everyone' and select the first section [3] from the drop-down menu. Enter the Due Date [2]. Click "+Add" to add the other section [4]. Select from the drop-down menu [3] and enter the Due Date. Depending on the number of sections, you may need to start typing part of the section number for it to appear in the drop-down menu.

Step 3: Import Mid-Semester Feedback Questions

Skip the "Save & Publish"/"Save" buttons at the bottom (for now) and select the Questions tab up top [1]. Click the "Find Questions" button [2].

Step 4: Select Questions to Import

There are two Mid-Semester Feedback question banks to choose from—the Online/Blended Mid-Semester Feedback Survey question bank includes questions from the face-to-face Mid-Semester Feedbank question bank.

Select the appropriate question bank [1]. You can select all questions from the question bank [2] or check the boxes next to the questions that you would like to import [3]. When you have finished selecting questions, click "Add Questions" to add them to your survey [4].

Step 5: Edit the Imported Questions (if necessary)

You can edit (pencil) or delete ("X") questions after you have added them to your survey [1]. When finished editing [2], don't forget to click "Update Question" to save your changes [3]. Note: some of the Mid-Semester questions contain instructions for the group of questions that follow.

Step 6: Save and Publish

Once you have finished adding/editing questions, click "Save and Publish" [1] for students to see and complete the survey. If you intend to come back and edit some more, click "Save."

Step 7: Done!
