Exercise 3b - Root Cause Analysis (Due before Day 3)

Follow the Root Cause Analysis process described in the Root Cause Analysis Reading (also posted on Module page) to determine the initial objectives, performance criteria, and constraints for Latinitas.

Base your Root Cause Analysis on the case in the handout for the in-class team exercise on Class Day 1 and the "additional background" provided on Day 2's module.

Include symptoms, a problem chain, and root causes, as explained in the Root Cause Analysis Reading. Propose at least one solution and list the objectives, performance criteria, and constraints for each solution you propose.

After you have compiled symptoms, problems, and root causes and linked them into a clear problem chain, you can move this into the Root Cause Analysis Template Download Root Cause Analysis Template

(also posted on the Root Cause reading) to finalize the process.  This template includes tips as well as the correct format. 

Turn in your finished assignment in Word or PDF format on Canvas via the link to the Assignment.

IMPORTANT: Please note that the examples posted in the readings are just for reference and are by no means a perfect example.  You should never copy/paste straight from the examples you are given. You are expected to think for yourselves and do better than example because the goal is not to learn how to copy others' work.   Please note you will have points deducted if we catch you copy/pasting other project work into your own.
